Aim of the study – to develop a system for monitoring disease course and assessing treatment effectiveness in patients with CML and MF based on immunological and molecular genetic tumor markers, to identify risk factors for disease relapse, and to elaborate a follow-up algorithm for patients in remission who continue or discontinue therapy. Methods – general clinical, cytological, molecular genetic, enzyme-linked immunosorbent assay, immunophenotyping by flow cytometry, and statistical data analysis methods. Results. A method for immunophenotypic identification of residual leukemic stem cells (CD34⁺CD38⁻CD26⁺) in patients with deep molecular response was developed and implemented. In 32 CML patients, the presence of leukemic stem cells in peripheral blood was detected, indicating persistence of the leukemic clone even during long-term tyrosine kinase inhibitor therapy. The combination of molecular monitoring using RT-qPCR with immunophenotypic analysis increases the sensitivity of minimal residual disease detection and supports its use when planning treatment discontinuation and during further follow-up. In patients with myelofibrosis, the development of cytopenias, immune disorders, and progression of bone marrow fibrosis associated with immunoregulatory cytokines (VEGF, TNF, IL-6, IL-8, IL-10) was confirmed. In patients treated with ruxolitinib for 6–12 months, clinical response was accompanied by decreased levels of VEGF and IL-6 and increased TNF levels, which may be used for monitoring treatment effectiveness and predicting therapy duration.
