Objective: To improve the results of kidney transplantation from suboptimal donors by improving the management tactics of the donor and recipient. To achieve the goal, the following tasks were set: · To provide a comparative assessment of the results of kidney transplantations from optimal and suboptimal donors. · To study the possibility of using biochemical markers of donor kidney damage to predict its function in the postoperative period. · To determine the feasibility and prognostic value of intraoperative donor kidney biopsies in suboptimal donors. To provide a comparative characteristic of morphological changes in the kidney from standard and suboptimal donors at the pre-transplantation stage using the so-called "zero" biopsies depending on the presence of risk factors in the donor: arterial hypertension, diabetes mellitus, progressive deterioration of hemodynamics on the background of high doses of sympathomimetics, increased serum creatinine level at the time of organ explantation, organ removal after cardiac (circulatory) arrest. · To investigate the morphology of the transplanted kidney from standard and suboptimal donors in the postoperative period by performing planned (protocol) biopsies of the graft. · Improve the diagnostic and therapeutic algorithm for conditioning potential donors in order to expand the donor pool by including suboptimal donors. · Develop tactics for managing recipients of a kidney from a suboptimal donor. As a result of the conducted research based on literature data and analysis of our own material (234 medical histories and outpatient charts of patients who underwent kidney allotransplantation at the State Institution "National Center of Hematology and Pathology named after O.O.
