The work highlights the key pathophysiological mechanisms of hemorrhagic shock, trauma-induced coagulopathy, hypothermia and metabolic acidosis as interrelated components of the “lethal triad” that determine the course of the perioperative period and treatment outcomes. The analysis of world literature demonstrates the evolution of approaches to infusion and transfusion therapy, from liberal crystalloid strategies to restrictive resuscitation with the priority use of whole blood or blood components in physiological ratios, early correction of coagulopathy and bleeding control. Special attention is paid to the role of anesthetic support as an active element of DCR, which directly affects the stability of hemodynamics, tissue perfusion and survival of the wounded. A comparative analysis of methodological approaches shows that the basic principles of safe anesthetic practice and patient monitoring were formulated in the International Standards for Safe Anesthesia Practice of the World Federation of Societies of Anaesthesiologists (WFSA). At the same time, these standards are of a general nature and do not fully take into account the specifics of combat trauma, massive blood loss and multi-stage medical evacuation in conditions of modern armed conflicts.
