Special attention was paid to modern principles of managing patients with massive blood loss, hemorrhagic shock, polytrauma and gunshot wounds of various localization. A comparative analysis of the organization of anesthetic care in military units of the Armed Forces of Ukraine, civilian hospitals, as well as in the armed forces of leading NATO countries (USA, Great Britain, Israel) was conducted. Modern approaches to infusion and transfusion therapy, anesthesia, homeostasis control and prevention of coagulopathies were evaluated. Based on the data obtained, key factors influencing the effectiveness of anesthetic support for the wounded in critical conditions were identified, as well as typical errors associated with limited resources, time shortage and imperfect standards. Based on the results of the study, a scientifically sound system for improving anesthetic care for wounded servicemen was developed, which includes algorithms for selecting anesthesia methods depending on the severity of the condition, optimized tactics of infusion and transfusion therapy for hemorrhagic shock. The developed proposals can be used to create updated clinical protocols and standards for anesthetic care in military medicine, aimed at reducing mortality and improving the results of treatment of the wounded.
