The object of study - 251 CLL patients, including 52 ionizing radiation exposed CLL patients. The rearrangements of variable regions of heavy chain immunoglobulins (IgHV) by polymerase chain reaction, sequence of received products and analysis using the IgBlast and IMGT databases and Italian CLL cohort of 1426 patients were conducted. Results. Some pecurialities of B-cell antigenic receptor in Ukrainian CLL patients were found: a high frequency of unmutated cases, decrease of IgHV3-23 gene usage, increase of IgHV1-58 and IgHV3-21 gene usage. The number of cases with stereotyped receptors was 39,8%. The distribution of cases with stereotyped receptors revealed a total absence of cluster #8 and low frequency of cluster#2. It is may be the effect of a potential environmental variable or pecurairities of genetic background. The homology of Ukrainian CLL cases with antibodies directed against viruses and bacteria (6.7%), auto-antigens (4.7%), immunoglobulins from patients with X-HІgM syndrome (7.9%) and B-cells from health donors (11.9%) were found. No differences in the frequency and distribution of stereotyped CLL cases depend on the influence of ionizing radiation were found. Eighteen new joint Italian-Ukrainian and 8 sole Ukrainian clusters with stereotyped sequences were identified, the homology of some of them with anti-viral, anti-bacterial antigodies and auto-antibodies were revealed.
