Objective: to improve the results of therapy of children with unfavorable malignancies of soft tissues (MST) by use of new methods for definition of groups of risk of patients using new programs of intensive polychemotherapy (PCT). Methods: clinical, angiographic, radiation, ultrasonic, molecular-and-genetic, immunological, immunocytochemical, statistical. Equipment: РАСScan" Весton Dikenson ", USA, 7300 Rеа1-Time РСR System "Applied Biosystems " USA, ATLHDI-5000 "Рhilips" USA, " Тоshibа Аsteon Super 4 ", Japan, "Stenoscop 9000 ", USA. Results: PCT with use of topotekan the direct positive result was received in 4 of 6 patients, remission was achieved in 3 of patients with MST of high risk. Maintenance of hemopoiesis with stem cells of peripheral blood (3 patients) twice reduces duration of leucopenia and thrombocytopenia of 4 grade,decreases frequency of complications connected with myelosuppression. High direct efficacy of intraarterial chemotherapy was proved (7 patients). Definition of genetic markers РАХ by 3/7 by the method of polymerase chain reaction improves stratification of patients (7 children with MST were revealed). On findings of this study in 41 children with MST suppression of cellular and humoral immunity and function of thymus was revealed. Disorders amplify after PCT and achievements of relapse.
