An object of study. Age-related cataracts, age-related cataracts, complicated by myopia (refractive, axial, lens-induced), age-related cataracts, complicated by zonular weakness age-related cataracts complicated by deterioration of the tear defficience syndrome. Survey 1170 patients (1314 eye) using clinical, instrumentation, functional and static methods. It has been found that although modern technologies of visual rehabilitation of patients with age-related cataracts have reached a high level such aspects as: predicting functional results regarding cataracts, complicated by myopia; astigmatism; optimizing calculation of optical power AC IOL, correction and prevention of post-operative dry eye syndrome remain relevant and require subsequent study. The method for calculating the optical power AC IOL in case of age-related cataract, complicated by myopia is analyzed and optimized. On the basis of analyses it has been proved that surgically induced astigmatism during phacoemulsification in case of its location in stronger refractive meridian depends on its type, and that there is a strong correlation between the diameter of the cornea and surgically induced astigmatism. It is proved that it is precisely this location that gives you an opportunity to obtain the highest uncorrected visual acuity in case of surgery of age-related cataract, complicated by myopia. The study found that in the 3.5 per cent of cases, the planned implantation of IOL during phacoemulsification endocapsular is impossible owing to lack of capsular bag (the main reason - pseudoexfoliative syndrome-73%); it was found that the AC IOL allows to reach sufficiently high visual functions, while deviation of post-operative refraction from targeted in patients with AC IOL using existing formulas of calculation of optical IOL power is clinically significant. HLB formula was developed to calculate the AC IOL power to achieve post-operative emmetropia in 95% of cases. We found that among patients with age-related cataracts, most patients have dry eye syndrome DES of varying degrees, and more than half of them have the available related pathology, it was found that phacoemulsification affect the existing DES by strengthening its manifestations and transition in heavier degree. It is proved that the most effective drug for the treatment of post-operative DES in beginning stage is Hypromellosae-P, in medium - Siccaprotect and in heavy -Systein. The results of r&d projects implemented in clinics faculty of Ophthalmology Depertment of FPGE LNMU named by Danylo Halytsckiy, a number of medical institutions of Ukraine. Projected assumptions-developed complexes of preventive activities and optimization models of refractive results of age-related cataract surgery may be recommended to the widespread implementation of the practical work of health facilities.
