With in the framework of self-consistent approach the macro- and microscopic susceptibilities, as well as memory and nonergodicity parameters, are determined as functions of the intensities of thermal and quenched disorders. The phase diagram is calculated that defines the domains of ordered, disordered, ergodic, and nonergodic phases.
