The object of study: 309 CLL patients, including 71 patients with influence of ionizing radiation in anamnesis. Methods of study: clinico-hematological, polymerase chain reaction, sequence of reaction products, statictics. The main results: the peculiarities of rearrangements of variable heavy chain immunoglobilun genes (IGHV) in CLL patiens, inhabitants of central part of Ukraine, in comparison with the others CLL cohorts were established: increased frequency of unmutated cases of disease (68.6%) and CLL cases in stereotyped B-cell receptors (38.5%), decreasing using of IGHV3-23 gene (2.3%), the absence of cluster 8 of stereotyped cases. New 18 joint Italian-Ukrainian and 9 Ukrainian clusters of stereotyped CLL cases were identified in cooperative study with Italian scientific group. These sequences were included in international database GenBank under submission numbers EF091900-EF091931, EF175384-EF175440, EF407822-EF407849, EF441744-441761, EF583659-EF583683, EU350410-EU350430, EU433859-EU433883, EU667594-EU667611, FJ694872-FJ694889, and HM173321-HM173333. It was confirmed that the mutation status of IGHV genes was the main molecular-genetic marker of CLL leukemic cells. Expression of unmutated IGHV genes was associated with advanced stage at diagnosis, high initial leukocytosis, bulky disease, development of autoimmune complications, short response on therapy, worse survival of patients. The ability of IGHV mutation status to stratify overall survival remained when applied to early-stage patients. The study of separate IGHV gene usage and stereotyped B-cell receptors had additional significance for CLL prognosis. With its help it was possible to determine probability of development of autoimmune hemolytic anemia, secondary tumors, for patients with unmutated IGHV genes - to reveal subgroup of patients with favorable (cluster 7) and unfavorable (cluster 6) prognosis. Among CLL patienrs, suffered due to Chornobyl NPP catastrophe, group of clean-up workers of April-May 1986 was revealed with mostly unmutated IGHV genes and some peculiarities of clinical behavior (increased frequency of solid cancers and Richter transformation).
