Purpose: to improve the results of treatment of patients with high-energy fractures of the thoracic and lumbar vertebrae by experimental-clinical substantiation of the concept of rational treatment based on the determination of residual fixation of the spinal motor segment, functional state of the spine and spinal canal. Research methods and apparatus: clinical, radiological and X-ray, magnetic resonance and computed tomography, statistical. Theoretical and practical results: Regularities of spinal deformation in the case of fractures of the vertebral and lumbar vertebrae under conditions of different volumes of destruction of anatomical structures were established. An algorithm for the treatment of explosive fractures and vertebral body fractures with distraction and rotation, taking into account the residual fixation of the spine, namely intact anatomical structures and their impact on the development of deformity, which allows to choose an effective treatment for this category of patients, improve its results and quality of life. Novelty: For the first time, a decrease in stress in the destroyed vertebral motor segments with an increase in the number of fixed vertebrae was determined. It is established that the use of intervertebral support leads to a decrease in stress in the posterior spine and metal structure. Implementation efficiency: The working classification of vertebral body fractures is developed, which is based on the principle of residual fixation of the spine, namely the signs of deformation of the spine and spinal canal, and allows to determine the criteria for transpedicular structures, length of spondylodesis, the need for interbody support and bone grafting.
