Aim was determining peculiarities of clinical current, heart remodelling dynamics in Chernobyl disaster survivors with prolonged EH and creating the measurements directed to the diagnostic and treatment optimisation. Methods: clinical inspection, 24-hour blood pressure (BP) monitoring and electrocardiography holter monitoring, 24-hour heart rate variability monitoring, echo-, dopplercardiography, exercise tests for definition of exercise tolerance, estimation of life quality. Results: regular peculiarities of adverse clinical current EH at the Chernobyl disaster survivors are occurrence and progressing left ventricle (LV) diastolic dysfunction and hypertrophy, increasing of a left atrium and LV cavity, deterioration of heart rate variability, reduction of exercise tolerance. Left atrium and right ventricle changes are appearing in EH duration more 5 years. Determinants of these changes were an index of loading by (BP) more than 50 %, high BP variability and increased speed of morning BP rising, infringement of BP daily structure. At prolonged EH patients within 1-3 years heart remodelling was characterised with infringement LV diastolic relaxation, increase of an index of the ratio left atrium size to end-diastolic the size more than 0,75, by increase of frequency LV hypertrophy higher than 2 levels. The EH current was accompanied by reduction general heart rate variability, which is characterised with statistical, geometrical and spectral parameters. Prevalence of sympathetic activation may be as marker of cardiotoxic effect. There was established, that significant heart ectopy revealed with electrocardiography holter monitoring, are connected with more expressed LV hypertrophy. At the Chernobyl disaster survivors with prolonged EH significant increase of intervetricular septum thickness, myocardium mass according ventricular ectopy was established. Reduction exercise tolerance, efficiency of its supply were already occurred at infringements of LF diastolic filling, progressed at LV hypertrophy, increase of a left atrium cavity. Correlation between long-term Chernobyl disaster factors, cardiovascular risk factors and peculiarities of EH clinical current was determined. Based on 3-yaers observation efficacy of compliance to antihypertensive treatment for reducing of heart remodelling, prevention of exercise tolerance and life quality lowering was established. Efficiency: determining the peculiarities of clinical current prolonged EH at the Chernobyl disaster survivors, creating recommendations and new diagnostic method will promote to prevention of EH complications, increasing of Chernobyl disaster survivors life quality.
