Aim was substantiation and developing diagnostic and treatment arrangements of CFH due CHD, direct to decrease it influence to Chernobyl disaster survivors state of health and capacity for work. Methods: Standard clinical examination, analysis of irradiation influence, 24-hour blood pressure monitoring, ECG monitoring, heart rate variability monitoring, dopplerechocardiography, bicycle ergometry. Results: Peculiarity and diagnostic value of clinical symptoms, law of reorganization of structures, functions, regulating of functioning of the heart activity, interrelations between the factors of failure and by the factor ischemic heart failure at Chernobyl disaster survivors (CDS) are determined. Normal ECG practically excludes ischemic CHF. The high sensitivity and specificity of echocardiography parameters for definition hypertrophy and left ventricle dysfunction is proved. Disturbance of myocardial function, development and current CHF and caused CHD are correlates with general duration of ischemia and quantity of ischemic events per day. The depression of a ST-segment, extrasystolia of high gradation were characterized high diagnostic sensitivity. The reduction of the majority of statistical and spectral parameters of heart rate variability, prevalence of low frequencies were established. The reduction of threshold power and volume of the executed work is shown at the CHF. Predictors of myocardial failure are determined. The techniques of the estimation of risk CHF and it progressing, algorithm of diagnostics and tactic of management the patients are developed. The insufficient efficacy of monotherapy and combination of hemodynamic drugs in treatment ischemic CHF was established. The directions of optimization of treatment are proved by influence on a wide spectrum of determinants its development and progressing. Metabolic active drugs and inhibitors angiotenzine converting enzymes were determined as a means of optimization of treatment. The inhibitors angiotenzine converting enzymes promote to antyischemic action, delay of development left ventricular hyperthrophy, positive influence to heart functions, heart rate variability. The antyischemic effect, reduction of ectopy heart activity, improvement of systolic and diastolic function and heart rate variability are determined after metabolic active therapy. During its using the exercise tolerance, volume and duration of exercise were increased. Based on results the guidelines are prepared, 2 expositions on the exhibition are submitted.
