Information × Registration Number 0207U002377, 0104U004249 , R & D reports Title Determination of mechanisms of formation and progress of chronic heart deficiency in children and adolescents with myocardial pathology popup.stage_title Head Bogmat L.F., Registration Date 25-01-2007 Organization Institute of Children and Adolescents Health Care of AMS of Ukraine popup.description2 Research object: chronic cardiac insufficiency (CCI) in adolescents aged 10-18 with myocardium pathology (MP).Objective: upgrading methods for diagnosing CCI in children and adolescents with MP on the basis of determining the character of changes in regulatory system, endothelial function, and in apoptosis processes.Methods: clinical, ECG , RECG, Doppler-echography, stress-echo CG, as well as biochemical, radioimmunologic, and immunofermental. Theoretical and practical results: in children with MP in the absence of complaints and CI clinical manifestations certain changes in the heart mophofunctional characteristics are formed, the changes manifesting themselves by an increase in the size of the left and the right ventricles, in a 31% decrease in the LV pumping and contractile functions and in rearrangement of hemodynamic correlations towards predominance of hypokinetic type, as well as in activation of neurohumoral regulation systems, which testifies to formation of CI subclinical stage. It has been proved that with a decrease in the myocardium functionality in adolescents there is observed an increase in angiotensin II level in, cytokinal system and apoptosis inductors activation. If has been shown that in MP certain findings of vascular endothelium function change: there are registered significantly higher findings of plasma prostacycline against the background of a slight endothelin I increase. It has been revealed that in 28% of patients there is observed an inadequate reaction of patients to physical load with a decrease in diurnal catecholamine excretion and in renin-angiotensin-aldosterone system findings which testifies to the presence of silent myocardium systolic dysfunction. It has been proved that significant changes in hemodynamic findings and activation of neurohumoral system are observed when LV ejection fraction is 55% or lower.Novelty: it has been first shown that in children with MP even in the absence of complaints and manifestations of congested CI there are formed certain changes which are the signs of CI subclinical stage. The borderline value (55%) of LV ejection fraction in adolescents for diagnosis of CI 1A stage has been substantiated. It has been proved that the factors of CI formation and development are: an increase in aldosterone and L6 interleukine in blood serum, a decrease in cAMP level and certain growth in LV systolic size. There has been worked out an algorithm for CI early diagnostics in children with MP, employing ECG load tests as its basic elements.Effectiveness: employment of diagnostic algorithm will make it possible to reveal CI initial signs in children and adolescents with MP at the early stages of pathological set of symptoms development and to prescribe an appropriate set of treatment and prophylactic measures.Application fields: pediatric, cardiology, pediatrics.
