Methods of study and instruments: clinical, laboratory, X-ray, ultrasonic, dosimetric; ROKUS (Russian Federation), roentgen simulator SimoView – 3000 (Siemens), universal dosimeter “Unidose E” (PTW, Freiburg), gammatherapeutic unit “Teratron Elit – 80” (MDS Nordian), linear accelerator of electrons “Mevatron” (Siemens), CT-scanner Somatom Plus 4 (Siemens), MP – tomograph “Magneton Visien” (Siemens), US units HDI – 5000 (Philips). Algorithm for use of diagnostic examinations of patients with malignancies and suspicion of metastases to skeleton has been developed and tested in clinic. In includes: MRT, CT, US+dopplerography, osteoscintigraphy, trepanbiopsy. Complex of therapeutic methods for the treatment of patients with malignancies and metastases to skeleton has been developed. It includes surgery, chemotherapy, radiotherapy, use of bisphosphonates and hormonotherapy, that alloved achieving 3 year survival 69,3 ± 5,09 %. It has been developed the method of three-dimensional planning of radiotherapy for the treatment of patients with malignancies and metastases to skeleton with taking into account heterogeneity of surrounding tissues and heterotransplant with use of three-dimensional planning system Theraplan Plus which gives anatomical information on the patient at a full extent and is a tool for optimization of chosen method of radiation. It has been developed a schedules of supportive therapy in complex treatment of patients with malignancies and metastases to skeleton which are based on colony-stimulating preparations (neupogen, granocyte, leukomax, eprex) and antiemetics (novoban, zofran). It allowed minimizing side effects of the treatment and its complications. It is proved that use of bisphosphonates in complex treatment of patients with malignancies and metastases to skeleton promotes recovery of bone tissue on a site of the lesion decreasing probability of pathologic fractures, It is proved that use of specific chemotherapy dependently on a type of primary tumor in the treatment of patients with malignancies and metastases to skeleton promotes decrease of amount of foci of metastases. It is proved that radiotherapy for malignancies and metastases to skeleton has both therapeutic and analgetic effect. It is proved that patients with malignancies and metastases to skeleton should have preventive puncture vertebroplastics for prevention of pathologic fractures. Indications for surgical treatment of patients with metastases to long bones, i.e. threat of pathologic fracture or its availability in ineffective chemotherapy are determined. It is proved that osteological changes of vertebral bodies or long bones revealed by CT or MRI are risk factors of pathologic fractures in patients with malignancies. Methods of low-invasive surgical interventions in metastases to vertebral column and long bones are developed. The analysis of the immediate, direct and remote results is performed using the developed methods. Social significance of a study consists in improvement of life quality and increase of survival of patients with metastases to skeleton. Scientific significance of a study consists in the development of the methods for complex treatment of patients with malignancies and metastases to skeleton.
