Three-port pars plana vitrectomy (PPV), as a modern technology, was conducted in all cases for the extraction of the lens luxated into the vitreous. The usage of this technique allowed to diagnose and to treat intraoperatively the posttraumatic retinal complications, if it was indicated, and also avoid complications which develops after traditional corneoscleral approach (more than 60,6 % complications). There were complications during PPV: subretinal hemorrhage (1 case) and hyphema (1 case), iatrogenic retinal tear (2 cases); after PPV: chroidal detachment (2 cases), retinal detachment (7,6 %). Visual acuity was saved and achieved in range 0,3-1,0 with aphakic correction in 99 of 138 (72 %) patients. The retrospective results' analysis of the extraction of the lens luxated into the vitreous body by a loop through thecorneoscleral approach and posterior chamber IOL suturing to the sclera showed frequency of such serious complications as retinal detachments (41,2 %), vitreous hemorrhage (17,6 %) and pupil block as a result of exudative reaction (17,6 %), endophtalmitis (5,9 %). In three cases the planned operation was failed, because of the vitreous hemorrhage. Secondary IOL implantation lead to the same complications except endophtalmitis each of one was observed in 7,7 %. The advantage of secondary IOL implantation is possibility to make sure in absence of some very serious complications, probability of which is determined by both severity of trauma and PPV.
