Goal: There was determining new approaches of clean-up workers after Chernobyl disaster after MI rehabilitation for restoration of heart remodeling, lowering of risk complications and exercise tolerance increasing. Methods: clinical, analysis of irradiation influence as well as others risk factors, Holter electrocardiography monitoring, Doppler-echocardiography, bicycle ergometry. Results: Based on studying of irradiation and traditional risk factors, that MI took precedence, peculiarities of changes of clinical parameters, heart remodeling, exercise tolerance, new approaches of clean-up workers after Chernobyl disaster after MI rehabilitation were determined. Rehabilitation protocols according to total risk level were created and its antiischemic anantiarrhythmic efficacy for regress of heart remodeling, exercise tolerance restoration as well as cardiovascular complications prevention was evidenced. In Q-IM clean-up workers after Chernobyl disaster were determined more ones >25 sZv irradiated. But risk factors before MI were registered in these cases. Next MI follow-up determined post-infarction angina pectoris, heart remodeling (predominantly left ventricle dilatation), heart rate variability lowering. Based on determining of the main factors and parameters those were before MI and its course peculiarities, there were created rehabilitation protocols in post-infarction clean-up workers after Chernobyl disaster. Clinical, heart remodeling and exercise tolerance dynamics during long-time rehabilitation in post-infarction clean-up workers after Chernobyl disaster according to protocols rehabilitation treatment were evaluated. Quantity and duration of angina pectoris events, as well as in 2 times silent ST-depression were reduced according to protocols rehabilitation treatment. Furthermore heart arrhythmia (ventricular andsupraventricular) lowering took place. After 2 year of rehabilitation in 85% MI patient's structure and function of heart left ventricle were stabilized, left ventricle hypertrophy didn't progress. Exercise tolerance was increased on 12.7 % and exercise duration was increased on 60 sec. As result 22 articles, 10 conferences reports, guidelines, new method were issues. Efficacy: Determining of management of clinical parameters, heart remodeling and exercise tolerance during long-term rehabilitation according to protocols facilitated to lowering of reinfarction, heart failure, hospitalizations in post-infarction clean-up workers after Chernobyl disaster.
