OBJECT: patients with local forms of pulmones bullous emphysema and with chest closed injury. OBJECTIVE: to decrease medical cours, reduction of after operative complications in patients with intrathoracic surgical pathology with different genesis via the usage of miniinvasive, reconstractive and plastic operations. RESULTS AND NOVELTY: the perfected treatment-and-diagnostic programme has been worked out for difficult intrathoracic closed injury. The necessity of differentiated choice of effective treatment-and-diagnostic actions has been grounded. The indications and contraindication to surgery, schedule times for different operations have been lay down. The new methods of ribs and fragment bones fixation have been proposed. The legacy methods have been improved for diagnostics and treatment of chest closed injury. The trauma after operation has been reduced from surgical aggression for pulmones bullous emphysema at the expense of use of axillary minithoracotomy technique. That technique provides favourable cosmetic effect and quick repair of functional activity of upper shoulder-girdle otth and after surgical aggression. The small extents of thoracic approach admits to abandon of ginglymoid suturing for ribs. FIELDS: thoracal and surgical departments of hospital.
