Object of investigation: 110 children of the first two years of life with epileptic seizures. The purpose of research: to study neurometabolic and structural disorders of the epileptic attacks and work out algorithms of early diagnostics and treatment of epilepsies in children of the first two years of life. Methods of investigation: Clinical examination of CNS status, general clinical examination, electroencephalographic mapping (EEG), magnetic resonance imaging (MRI) and magnetic resonance spectroscopy (MRS), neurosonography (NSG), testing gamma-aminobutyricum acid (GABA) in the serum, genetic tests. Were done investigations in children of the first two years of life with epileptic seizures using modern methods of neurovisualisation and in accordance with pathogenetic mechanisms was established comprehensive treatment which uses modern anticonvulsants in combination with neuromethabolic drugs based on by elaborated individual schemes. Dynamic observation and examination of these children allowed to evaluate the effectiveness of the elaborated schemes while on the treatment of epileptic syndromes and epilepsies (frequently with pharmacological resistant) in newborn and children of early age. The novelty of this work is that in the first time comprehensively were defined the principal starting mechanisms of epileptic pathogenesis in newborn and children of the first two years of life by the definition of the role of structural (using modern methods of neurovisualisation - NSG, MRI) and metabolic disorders (by testing braking and exciting neurotransmitters in the brain, visualization of the brain status). Was proven that in young children pharmacological resistance in epileptic syndromes and epilepsies based on anomalies of the brain in the form of congenital defects of CNS and structural disorders due to hypoxic-ischemic, traumatic and infectious causes originated in pre-and perinatal periods. The scientific and theoretical value is to improve methods of early diagnosis of the epileptic syndromes and epilepsies in newborns and children of the first two years of life, develop the algorithms of early diagnosis and pathogenetically proved schemes of treatment of various forms of epileptic syndromes and epilepsies in above mentioned children using modern anticonvulsants in combination with neuromethabolic drugs and justify their effectiveness. This would reduce the social disadaptation and disability of children with epileptic syndromes and epilepsies. Based on the work were published 15 scientific articles, done 24 talks at conferences and congresses, including international (Germany, China, Hungary). Obtained 2 patents and filed useful model application № U200907043 on diagnosis and treatment of epileptic syndromes and epilepsies in newborns and children of the first two years of life.
