The object of investigation: during pregnancy and childbirth in 160 pregnant women with congenital heart disease and 50 healthy pregnant women. Purpose: to reduce the frequency of obstetric and perinatal complications in women with congenital heart disease through the study and development of prenatal and perinatal preparation of the fetus based on identified characteristics of the pathogenesis of complications, and factors contributing to their emergence. Methods: statistical, instrumental, morphological, immunohistochemical, radioimmunoassay, flyuorometric, questioning. Used equipment: ultrasonic device "MEDISON", cardiotocography "Kranzbuhler", fluorimeter EF-ZMA, radiometer GAMMA 12. Based on the analysis of maternal mortality in Ukraine for 15 years period it was stated, that congenital heart diseases possessed the first place among the mortality causes in pregnant and postpartum women with cardiac pathology (23,7%), and among them the majority are the women with pulmonary hypertension (77,4%). Congenital heart diseases have the negative impact on the pregnancy, delivery period, being the cause for the development of obstetric and cardiac disorders. It was proved, that in this pathology the significant reduction of life quality, tolerance to physical burden, disorders of neurohumoral regulation in system mother-placenta -fetus-newborn. The role of disorders of stress-associated system in the development of obstetric and perinatal complications was stated, as well as the role of changes of ratio between concentrations of progesterone an estradiol and the expression of their receptors. It was worked out the system of antenatal preparation and complex therapy of pregnant with congenital heart diseases, engaging the physical burden, psychological correction, measures for the reduction of maternal and fetal hypoxia, normalization of fetal-placental complex function, interrelations in system of neurohumoral regulation and spa-rehabilitation. The efficacy of the suggested system was proved by the improvement of life quality, normalization of psychological state of patients, enhance of toleration for physical burden, improvement of the function of cardiac, obstetric and perinatal disorders. The scientific know-how of this work: for the first time the new chains of pathogeny of the mechanism of the maternal and fetal disorders development are being clarified, on this background the new methods of prognostication, therapy and rehabilitation, antenatal preparation and perinatal fetal care are being worked out in women with congenital heart diseases. The results of scientific research are reflected in 15 publications in scientific journals and collections, three reports at scientific-practical conferences and two protocols of Ministry of Health Care of Ukraine.
