Purpose of the investigation- to get new knowledge about development of the operated hip joint in children with dysplastic pathology, to ground, to develop and to improve methods of the surgical treatment for the results improving and to decrease disability. Methods of the investigation- clinical, radiological, roentgenological (including computer tomography), ultrasound, biomechanical, morphologic, statistical. It is found that surgical treatment of the hip joint in children have compilations and recurrences rate in 41%-84%. However the efficacy of the hip joint surgical treatment is quite high. Main causes of the complications are: diagnostic; underestimation of the pathology severity (22%); recommendation neglecting - 18%, tactical - 19%, technical-16%, organizational-methodic and material - 25%. It is defined dynamic of the following data: rentgenological, sonographic, dopplerograhic, biomechanical, morphological, biochemical about development of the dysplastic hip joint before and after operation; it is defined the place of the integral radiological index decentration of the femoral head as one of the first sign of the hip joint instability, several types of the decentration are separated;at first time sagittal hip-spinal alignment has been assessed in children with congenital hip dislocation, the relevance of this paramentres has been showed in revision surgery planning.With the method of final element analyses strain-stress distribution was assessed in the hip joint after the different pelvic osteotomies. This gives the opportunity to develop new osteotomies and to specify indication for their using. Clinical approbation of the proposed operation was done. New system of the postoperative rehabilitation has been proposed in children with congenital hip dislocation after revision surgeries. Dysplastic syndromes of the hop joint are separated, new classification of the surgical in congenital hip dislocation and subluxation has been develop, it is proposed typical combination of the modules. New algorithm of the diagnostic and treatment of congenital hip dislocation and subluxation with recommendation for using it is proposed. Using of the algorithm allowed to obtain 52% of good results, 28% of satisfactory results and only 21% of unsatisfactory results after revision surgeries.
