Aim: to improvement of diagnosing, forecast and clinical course subject to role of apoptosis and molecular disturbance in CMPD patients suffered from Chornobyl accident. Methods of the study: clinical, morphological, histological,cytogenetic, molecular, immunological and mathematical. Results: examined 220 patients of basic and opposite groups with CMPD: 172 CML patients (17 suffered from Chornobyl accident) and 48 osteomyelofibrosis patients (29 suffered from Chornobyl accident). It was shown that inhibitor tirosinekinase treatment improve CML course, but qualitative changes in blood and bone marrow granulocytes is maintenanced in patients suffered from Chornobyl accident. Abnormal megakariocytes differentiation in CML patient was revealed. Ph-chromosome and b3a2 transcript (79,6%) are typical for CML patients. Small part of patients have had b3a2 and ela2 coexpression that indicate polyclonality of the pathologic process. Lack of inhibitor tirosine kinase response correlate with CD95 high level expression; high level expression Bcl-2 protein disable involvment BCR-ABL-positive cells in apoptosis; it was found correlation between bone marrow CD38+ cells and number Ph-metaphases that could be prognositc criteriaes for CML course. Osteomielofibrosis patients suffered from Chornobyl accident have had apparent changes of bone tissue and high level fibrosis. Peculiarity of platelet's aggregation of patients suffered from Chornobyl accident cause to individual therapeutic approach for prevantion of trombosis and hemorrhagic complication. It was found role of apoptosis and prognostic criteria in CML course under tirosinekinase inhibitor treatment. The pathogenetic background of diagnostics was justified, course prognostics and therapeutic approach development accounting the complex of clinical and haematological features CML course of the apoptosis progress and molecular-genetic disturbance on different time of diseases was showed.
