Objective: to develop optimum monitoring program for patients with cardiovascular system diseases and to ground scientifically approaches to the diferentiated treatment and prevention of thrombotic complications on the base of studying disorders in the hemostatic system associated with the dificiency or dysfunction of plasma hemocoagulation factors, hyperhomocysteinemia, genetic thrombophilia factors. Methods: clinical examination, biochemical, clotting tests, aggregatometry, genetic, instrumental (ExoCG, ECG, reactive hyperemia test), arteriography, phlebography, statistic methods. The prevalence of geneticodetermined and acquired thrombophilia risk factors in 180 practically healthy persons and in 488 patients with vascular disorders were studied. The correlation of prothrombogenic mutations with metabolic state, endothelial dycfunction, hemostatic system sites was assessed. The patients with cardiovascular system disorders were found to accumulate carriers of genetic abnormalities, hemocoagulation and metabolic disorders, which have complex character, are associated with high rate of thrombosis recurrence, thromboembolic complications and progressing endothelial dysfunction. Experimentally, the mechanism of hyperhomocysteinemia effect on hemostasis system was grounded, thrombocyte hemostasis system sites were shown to be the most sensitive to high homocystwin level effect. The main principles individual treatment for patients with cardiovascular system disorder regarding metabolic risk factor profiles and genetic abnormalities have been formulated. Practical value. The algorithms of diagnostics, prophylaxis, thrombophilia corrections for patients with vascular diseases were made, the recommendations on invalid rehabilitation depending upon combination of genetic and metabolic risk factors of thrombofilic syndrome were developed.
