Objective: the improvement of prophylaxis and treatment of postoperative painful syndrome of lower extremity stumps on the ground of deepend studying its pathogenesis and differentiated approach depending on disease forms, clinical course features, prothesis character, physiological and biochemical investigation data.Object: Forms of postamputation pain in amputated lower extremity stumps. 815 pftients with amputation lower extremity defects were examined. The examination allowed to study freqency and structure of different clinical postamputating pain forms. Four new forms of local painful syndrome of amputated lower extremity stumps were distinguished; osteomyodystrophic syndrome, fibula stump syndrome, tunnel neuropathy, chronic compartment syndrome. To objectivize painful syndrome clinical, roentgenologic, morphological, biochemical methods, measuring intraosteal and subfascial pressure, electroneuromyography, non-invasive spectrophotometry, barostesiometry, tensioalgometry, oxigen tension definition in soft tissue were used. Frequently amputation proved to be the intermediate stage in painful syndrome ccurence and starting moment is a longterm hypokinesia prior to amputation. Such forms of local painful syndrome as chronic compartment syndrome, tunnel neuropathy, fibula stump syndrome were determined to develop under the influence of prothesis on the background of the amputation technique defects and local circulatory disorders, but osteomyodystrophic syndrome is due to the amputation cause, amputation plasty character, partificularities of the postoperative period course and prothesis terms.Theoretical significance: elucidation of postamputating painful syndrome formation algorythm and argument methods of prophylaxis and treatment. Practical importance: the obtained results help to accom-plish differentiated approaches to prophylaxis and various forms of postoperative painful syndrome therapy. Offered preventive and therapeutic methods for painful syndrome treatment enlarge the spectrum of routine methods removing pathogenetic factors to receive positive results. These results regarding marker effect of lipid peroxid oxidation, oxidated protein destruction, C-reactive protein and pro-inflammatory cytokins on intensity and painful syndrome course may be used not only in surgery but in MCE practice (medicosocial examination) for pain objectivization as well.
