The "traditional" methods of treatment were applied in a control group (36). The personal methods of correction of intra-abdominal hypertension and abdominal compartment - syndrome were applied in a basic group (51), bacterial translocation and internal intoxication. Information was established about the dynamics of intra-abdominal pressure, disbiotic processes in intestinum in patients with acute intestinal ileus and peritonitis, enteral insufficiency. It is concluded that for all of patients with intestinal ileus and peritonitis intra-abdominal hypertension and surplus bacterial colonization of all of it departments take place with predominance of gram-negative aerobic microflora. It is established, that frequency of development of bacterial translocation and internal intoxication depended on the quantity of intra-abdominal pressure and bacterial contamination of intestinum. The complex program of intraoperative therapy and conduct of postoperative period was developed and applied in the basic group of patients, this program which is based on the use of probe decompression of intestine, intestinal lavage with the active aspiration of intestinal content, directed on the reduce intraintestinal and intraabdominal pressure. Complex administration of antibiotics and other preparations in intraintestinal, intraportal, intramesenterial ways provided suppression of bacterial translocation. At the same time decompression of urinary bladder was carried out with monitoring of intra-abdominal pressure and early stimulation of peristalsis of bowel. The application of this program allowed to improve the results of operative interference, reduce the amount of postoperative festering-inflammatory complications in 3,3 time, to postoperative lethality in 3,5 time, terms of stay inpatient in the average on 2,6 day, and expenses on treatment of patients with intestinal ileus and peritonitis.
