Object of the study - occlusive and stenotic lesions of cerebral arteries in patients with arterial hypertension, who are considered as a candidates for operative revascularization. Aim of the study - to develop measures aiming prevention of surgical complications in patients with arterial hypertension who has stenotic lesions of cerebral arteries and to evaluate its effectiveness. Methods of the study: noninvasive - ultrasound dopplerography, computer tomography, magnetic resonance tomography, electro-encephalography; invasive - cerebral angiography. In the study 102 patients with arterial hypertension and with occlusive-stenotic lesions of cerebral arteries who needed operative revascularization. There were following types of operative treatment: endaterectomy (19 cases), transluminal angioplasty and stenting (83 cases). The factors which increase the risk of carotid endarterectomy were under the study: multiplicity of stenotic lesions and occlusion of contralateral carotid artery. Those causes lead to insufficiency of collateral blood flow do corresponding vascular territory and hence raise the risk of intraoperative ischemic stroke. Increased risk of endovascular interventional procedures could be caused by deformations of carotid arteries as well as by peculiar configuration of aortic arch. Depending on the features of cerebral arterial stenotic lesions, on the characteristics of cerebral blood flow and accompanied somatic lesions we established indications for definite types of surgical management (open or endovascular) which carries low risk of intra- and postoperative complications.
