A.Romodanov, AMS of Ukraine" popup.description2 The study provided new data about the topographic and anatomical relationships between structures in the craniovertebral anomalies such as Arnold-Chiari malformation and syringomyelia; established radiological signs of cystic intramedullary compression dynamics of liquor circulation disorders in craniovertebral region, its influence on the formation of syringomyelia; developed pathogenetically reasonable surgical techniques eliminate the medullary compression and restoration of liquor circulation in the craniovertebral region. From a practical point of view developed in the process of implementation of scientific topics diagnosis and surgical treatment of patients with syringomyelia and Chiari anomaly, have led to increased efficiency of surgical treatment of these patients, significantly fewer postoperative complications and improve quality of life of patients. The object of study: patients with Arnold-Chiari anomaly and syringomyelia. Objective: To increase the effectiveness of surgical treatment of patients with Chiari anomaly based on the study neuroimaging data, developing tactics and techniques of surgical interventions. Research Methods and equipment: 1.General clinical examination of patients, laboratory research methods. 2.Electrophysiological methods. 3.Neuroimage methods: radiography, MRI, CT scans, spinal selective angiography, venospondilografiya, MRI-angiography. 4. Intraoperative study of topographic features of the anomaly Chiari malformation, syringomyelia. 5.Micrograph video surgeries, analysis of the features of surgical technique. 6. Statistical analysis of point scoring of neurological manifestations before and after surgery. Results and novelty: 1.Studied clinical signs of the anomaly Chiari malformation, syringomyelia. 2. Refined CT, MRI features of Chiari anomaly, marked clinical types of Chiari anomaly. 4. Developed a diagnostic algorithm for anomaly Chiari malformation, syringomyelia. 5. First developed surgical treatment of syringomyelia and Chiari anomaly, depending on their clinical radiological types. 6. Improvements to existing techniques of bone and shell decompression anomaly Chiari, syringomyelia. For the first time developed a method of combination of bone and shell, internal decompression of the craniovertebral junction and reconstruction of the great occipital cistern. 7. Studied the immediate results of treatment of Chiari anomaly, the efficiency of the developed methods of surgical treatment. 8. Studied long-term outcomes Chiari anomaly. Demonstrated regression of siringomieliticheskih cysts, restoring liquor circulation in the late period. Efficiency of implementation: the implementation of this work has significantly improved outcomes in patients with Arnold-Chiari anomaly on the basis of the developed surgical techniques and create precise algorithm for such treatment, which are widely used in specialized medical institutions of Ukraine.
