The purpose of the work to learn pathogenetic mechanisms of damage of genesial system at patients with a genital endometriosis on the basis of studying of features of clinical course, an assessment of anatomo- and morpho-functional states of members-targets, features of an endocrine homeostasis, state of the general and aboriginal immunodefence. Research methods - clinical, apparatus, bacteriological, bacterioscopic, virologic, immunologic, immunoenzymatic, endocrinologic, histological, immunohistochemical, statistical. The used equipment and gauges - immunoenzymatic analyzer Stat Fax 303 plus, firm test systems "Novum diagnostica GMBH" (Germany), UBI Magiwell and Calbiotech (USA). The work represents the theoretical substantiation of the concept of pathogenetic mechanisms of damage of genesial system at patients with a genital endometriosis where basis disturbances of processes of an invasion, apoptosis and a cellular proliferation of frames endometrium against imbalance of the hormonal and the immunological homeostasis, inflammatory process are presented. Substantial increase of basal secretion of Gonadotropinums and their is stable high concentrations in the middle of a menstrual cycle, depression ovarian reserve of ovaries are taped. There is no immune answer to an ectopic invasion endometrium; leaders are disturbances of local cellular immunodefence. Dependence of an expression of receptors to and to progesterone in ovaries from their morphological features, inhibition of regulation of apoptotic activity are taped as well. The expression matrix metaloproteinazy - 1 phylum in a cytoplasma of an epithelium of a side and cellular structure of contents endometrioma synchronises with growth of an invasiveness of histic substrate, disturbance mechanisms expression of receptors to oestrogens and to progesterone, of apoptotic activity. Girls with disturbances of a menstrual cycle is bunch of risk of development in a genital endometriosis. The choice of agents of hormonal therapy of an endometriosis requires an individualization. Surgical treatment of endometriosis at women of genesial age should be maximum organopreserve. Application of advanced stage-by-stage tactics of treatment of patients has allowed to enrich genesial function in 86,6 % and to recover generative function in 57,3 % of women. The received results of researches are displayed in 25 scientific articles in magazines, 6 performances at scientific conferences, 3 patents for beneficial model, 3 circulars.
