Myocardial express necropsies from 36 patients (28 males , 8 females, age range 39-79) who suffered mainly from HT and died from ST-elevation myocardial infarction (MI), complicated by acute heart failure (HF), were examined by electron microscopy. The second group was represented by 10 patients with myocardial biopsy, obtained during endoventricular plasty of chronic postinfarction aneurism wall or performing coronary artery bypass grafting (CABG), which were examined by electron microscopy. Cardioprotective agents (perindoprili, carvediloli, spironolactoni, trimetazidini) were examined in control group of 50 patients suffering from HT and CHD. It was revealed that causes myocardial cell (MC) and endothelial cell (EC) hypertrophy, their structural and functional heterogeneity, preceding their hibernation and apoptosis. Left ventricle diastolic dysfunction in hypertensive patients is partly associated with MC hypertrophy and hibernation while systolic dysfunction is associated with MC and EC hibernation and apoptosis. MI except necrosis exacerbate cellular hibernation and apoptosis in different zones of the left ventricle resulting in progression of HF. MC hibernation and apoptosis leads to the loss of myocardium mass, systolic dysfunction and HF progression. Neovascularisation of the chronic aneurism wall reveals to be functionally unsufficient for hibernated MC reperfusion , the same as pharmacologocal treatment with cardioprotective agents of the infarction zone appeared to be with low effect. Since MC hibernation and apoptosis is revealed in patients with HT and MI, revascularization ( stenting, CABG) should be performed at the earliest time as possible. Еxept that all patients with HT need the prescription of cardio- and vasculoprotective drugs (perindoprili, carvediloli, spironolactoni, trimethazidini) preventing myocardial cell apoptosis and HT. Such therapy it considerably improves left ventricle ejection fraction, leading to the revitalization of hibernated myocardium.
