Aim of the research: To improve complex diagnostics and therapy of chronic dermatosis (psoriatic, acne diseases) and sexually transmitted diseases of non-gonorrheal etiology considering peculiarities of clinical course in patients of different age groups by means of analysis of results of clinical laboratory analyses, established correlative interconnections between them, which will allow to enhance effectiveness of treatment and broaden notion about pathogenesis of the above-mentioned diseases. To investigate character, possible causes of appearance and peculiarities of psoriatic disease and acne disease course in patients of various age groups. To determine incidence and degree of severity of psoriatic disease, acne disease, sexually transmitted infections. To substantiate terminology pathogenically for the description of stages of changes in AP. To elaborate clinical classificatory signs for determination of psoriatic disease phenotype. To improve questionnaires of examination of patients with psoriatic disease, sexually transmitted infections. To track specific pathognomonic signs of AP and elaborate searching algorithm of its differential diagnostics. To investigate presence of disturbances in functioning of some internal organs and systems in patients with psoriatic disease, acne disease. To study character of changes of some biochemical indices, which represent condition of hepatobiliary system, antioxidant activity of the body in psoriatic disease. To establish possibility of correlative connection of biochemical indices with a character of the course of cutaneous or articulation psoriatic process. To investigate degree of disturbances of lipid metabolism oxidation, condition of skin microbiocenosis, intestines and microelement composition of hair in patients with acne disease. To identify agents of sexually transmitted infections. To propose approximate schematic model of psoriatic disease pathogenesis and its treatment as a systemic disease on the basis of complex laboratory (biochemical and microbiocenosis of the intestines) results. To elaborate pathogenically substantiated differential method of treatment of patients with acne disease with various degrees of severity of the course using methods for correction of metabolism, microelement metabolism, microbiocenosis of the intestines and skin. To evaluate diagnostic prognostic importance of degree of metabolic changes in patients with psoriasis and activity of the course of psoriatic process during recommended therapy. To investigate degree of correction of impaired indices of lipid metabolism, condition of skin microbiocenosis, intestines and microelement composition of hair in patients with acne disease. By means of identification of agents in smears from genitourinary tract of patients with sexually transmitted infections, to elaborate algorithm of their differential diagnostics and improve complex therapy in order to obtain complete clinical recovery. To evaluate therapeutic effectiveness of the suggested methods of treatment of chronic dermatosis (psoriatic, acne disease) and sexually transmitted diseases of non-gonorrheal etiology in comparison with the basic one. To elaborate and implement into practical dermato-venerology pathogenic schemes of treatment of psoriatic disease, acne disease, sexually transmitted infections. Methods of investigation: clinical, biochemical, microbiological, bacteriological, roentgen fluorescent, computer bacterioscopy, statistical. It has been established that at present in Lviv region severe forms of psoriatic disease have become widespread. Dominant venereal infection is trichomoniasis, acne has become considerably spread among teenagers (to 80-90%). It has been proved that in 80-89% of patients with psoriatic disease and acne disease causes of clinical progress of cutaneous process were numerous inflammatory processes of internal organs, impairment of biocenosis. Clinical diagnostic criteria for determination of psoriatic disease phenotype and algorithm of differential diagnostics of AP have been elaborated. Questionnaire for patients with psoriatic disease, sexually transmitted infection has been improved. Pathogenically substantiated terminology for the description of stages of changes in AP has been suggested, including: psoriatic enthesopathy (periarticular impairment, that is proved with ultrasound diagnostics, MRI), psoriatic arthritis and dominant psoriatic arthritis (bone changes are visualized on roentgen). Specific pathognomonic signs of AP have been tracked: impairment of joints and osteolysis. Diagnostic importance of parallelism of the course of cutaneous and articulation syndrome in AP has been proved. Expediency of determination of a number of biochemical investigations has been pathogenically substantiated for evaluation of disturbances of POL-AOA , hepatobiliary system in patients with psoriasis, activity of clinical course of dermatosis. On the basis of results of complex biochemical investigation a schematic model of pathogenesis of psoriatic disease as a systemic disorder has been suggested. Lipid imbalance has been detected in most patients with acne disease. Alterations in skin microbiocenosis in 86.7% of patients and dysbacteriosis in 83.7% have been established, regardless the degree of severity of acne disease. Lack of microelements zinc and selenium has been detected in hair of 80% of patients with acne disease. Agents of venereal genitourinary infections have been identified. Differential method of complex therapy of psoriatic disease has been suggested with controlling of PASI index, using corrective medications (enerliv, manax, symbiter) for normalization of indices of functional ability of the liver, POL-AOA and topical medications (calcitriol + betamethazon valerat 0.1%). It is recommended to include into therapy of patients with acne corrective medications: for elimination of intestinal dysbacteriosis - Hilak-forte; normalization of lipidogram indices - Epadol and microelement condition - Vitam. As a topical application "Uhryn" and "Zinerit" should be used, depending on degree of severity of the disease. Algorithm of differential diagnostics of sexually transmitted infections has been elaborated and scheme of complex therapy of patients with chronic urogenital chlamydiosis (azymed in combination with manax, symbiter); trichomoniasis (ceknidazol); in marked bacterial vaginosis and viral invasion (suppositories of henferon, Eberon-a (interferon-a2b) in injections). In addition to proposed schemes of sexually transmitted infections treatment, Symbiter and gel Intim-Aid are recommended for normalization of biocenosis of digestive tract and pH of the vagina. Clinical laboratory effectiveness of recommended pathogenic schemes of therapy of patients with psoriatic disease, acne disease, sexually transmitted infections has been proved in comparison with the basic ones, that allows to recommend them for implementation into practice both for treatment and prevention of relapse of the diseases.
