Danilevsky Institute of Endocrine Pathology Problems of Academy of Medical Sciences of Ukraine" popup.description2 The object of the investigation is hormonal-immunological and metabolic homeostasis in patients with combined endocrine pathology, DM and AIT. The purpose of the investigation is to work out pathogenetically based treatment on the basis of studying the features of polyendocrinopathies clinical proceeding (diabetes mellitus (DM) and autoimmune thyroiditis (AIT)) and patients' immunological status. Methods of investigation are retrospective analysis of results of investigation in patients with polyendocrinopathy, DM and AIT. It has been revealed that the combination of DM with AIT is met in 28,3 % cases in all of observed patients with DM. In all examined patients with polyendocrinopathy (DM with AIT) women prevailed. The combination of DM type 2 with AIT occurs more frequent, than DM type 1 with AIT. The carbohydrate exchange in patients with DM and polyendocrinopathies appeared to be decompensated, and the lipid levels in patients with DM type 2 with AIT turned out to be higher than in patients with DM only. The changes in Т-cell component of the immune reply were revealed, thus level of T-suppressors was for certain below in patients with DM type 2 with AIT comparing to the patients with DM type 1 with AIT. The increase of NK-cells in patients with DM type 2 and DM type 2 with AIT in comparison with the patients with DM type 1 and DM type 1 with AIT can be related to the age peculiarities of patients. Expression of Fas/CD95 in all of groups was within the norm limits, what can be the display of possible compensatory mechanisms activating if to take into consideration the IL-4 level increase. When evaluating the markers of inflammation in patients with DM type 2 and AIT the actual increase of fibrinogen IL-6 and IL-1 level was revealed comparing to the patients with DM type 1 and DM type 1 with AIT. Direct cross-correlation connection is set between BMI and fibrinogen level in patients with DM type 2 with AIT. Revealed changes in the immune system and cytokine levels in patients with DM type 2 and with AIT indicate a greater activity of chronic inflammatory process in the presence of abdominal obesity and metabolic violations which may cause autoimmune pathology. Practical results: the necessity of thyroid pathology screening of all patients with DM, especially type 2 has been proved. The therapy schemes of combined endocrinopathy taking into account metabolic and immunological changes are suggested. Application area: Health Care.
