The limit plastic operations which can be carrying out at patients with defect of soft tissue of a head and a neck is defined. It is defect of a certain anatomo-functional site of a head and a neck which occupies more than 15 % of its area. Adequate replacement of extensive defects of soft tissue of head and a neck probably only at use of microsurgical transplantation of complexes of tissue. Such surgical operations on replacement of extensive defects of soft tissue of head and a neck, as use of complexes of tissue on perforant vessels and prelamination complexes of own tissue with their microsurgical change are perspective.
