We considered in de-tail the relationships of malfunction of regulatory systems with changes in intracar-diac haemodynamics and function of vascular endothelium. During the analysis of distribution of genotypes and alleles of type AT1R genotype we established for the first time for subpopulations of Vinnytsia region that the pre-dominance of the AC and the allele C among patients with heart failure complicated the course of the essential hypertension. We determined that the AT1R gene poly-morphism is associated in essential hypertension with the remodeling of the structure and function of the heart and, respectively, with the likelihood of heart failure. For carriers of allele C the incidence of heart failure was almost twice higher than for car-riers of allele A. Above that in carriers of AC genotype the risk of heart failure with systolic dysfunction was significantly higher. We established at first that the presence of genotypes AC or CC was associated with more significant pathological changes in functional state of the vascular endothelium. We found for the first time that the frequency of the genotype Pro / Pro and Pro allele is lower among persons with essential hypertension, compared to the healthy persons. The presense of the polymorphism PPAR-? was associated with structural and functional parameters of intracardiac and systemic hemodynamics, which manifested itself in carriers of genotype Pro / Ala and Ala / Ala with increase in myocardial mass, increase of the heart cavities and the deteriorating of transmitral blood flow.
