The object of study: 327 patients with chronic lymphocytic leukemia (CLL) and 327 individuals without oncohematological pathology. Methods: the determination of rs6449182 CD38 gene polymorphism by polymerase chain reaction and restriction of products, clinical, statistical. The main results: the frequencies of rs6449182 polymorphisms for CD38 gene were determined among patients with CLL and persons without oncohematological pathology (control group) who are constantly living in Ukraine. It was found that distribution of polymorphisms in controls did not differ depend on radiation anamnesis, gender, and age of observed patients and corresponded to the others groups of Caucasians. The impact of these polymorphism on the risk of CLL in patients exposed to ionizing radiation (IR) did not found. Some differences were revealed among IR non-exposed CLL patients with CLL depending on the place of their residence, namely increasing the GG genotype carriers (28.6%) among residents of rural areas compared to city dwellers (10.1%). The genotype GG was associated with unfevorable features of CLL: higher initial leukocytosis and more frequent hyperplastic syndrome at diagnosis, more rapid progression of the disease, early beginning of treatment, increased risk of Richter transformation (OR = 3.159; 95% confidence interval 1.283-7.77; p = 0.009), a shorter period of overall survival regardless of mutational status of IGHV genes. In the control group of non IR-exposed individuals the association between genotype GG and some parameters of lipid metabolism was found (increased concentrations of serum lipoprotein, cholesterol, triglycerides). In the control group IR-exposed individuals GG genotype was associated with an increased risk of developing of diabetes mellitus, type II (OR = 3.11, 95% CI 1.26-7.65, p=0.011).
