Danilevsky Institute of Endocrine Pathology Problems of Academy of Medical Sciences of Ukraine" popup.description2 Subjects - patients with somatotropinoma and somatomammotropinoma Aims - taking into account features of demonstration, a clinical current and on the basis of results of the analysis of the remote results of treatment of tumours of a hypophysis (somato- and somatomammotropinoma) to create national medical-diagnostic algorithm of conducting the given category of the patients which observance would promote increase of efficiency of therapy, qualities of life and to increase in life expectancy of patients. The 97 patients with somato- and somatomamotropinomas were under investigation. In 96,9 % of patients the clinical and hormonal control of disease was insufficient. Effects of hypersomatopropinemia on carbohydrate metabolism consist in the development of moderate resistance to insulin action and compensatory hyperinsulinemia to support glucose homeostasis. As a result of that intolerance to glucose was detected in 38,9 % of patients with acromegalia. Some lipid's spectrum abnormalities, independent from adenoma's activity and sex, were detected in 40,4 % with acromegalia, including highly atherogenic secondary hyperlipidemias have been revealed in 34,7 % of patients. Thyroid gland's abnormalities were found out in 85,6 % patients with acromegalia: diffuse and nodal goiters were associated with hypersomatotropinemia. Disturbances in myocardial structure and function depended on patient's age and duration of disease. The main predictors of elevation of ventricular ectopic activity were arterial hypertension and structural alterations in the left ventricular myocardium. The normal myocardium geometry or circular type of the left ventricle remodeling are specific to the middle (young) age patients. In spite of intact systolic function of the left ventricle the majority of patients with acromegalia are suffering from diastolic dysfunction which severity depends on age and serum growth hormone level. The algorithm for prediction of myocardium type remodeling to anticipate the course of disease and timely undertake pharmacologic correction was proposed. It was done background to introduce in medical practice the acromegalic patient's register that is a flexible tool enabled us to spread information about prevalence and incidence of acromegalia, patient's age and sex, disease complication, effective treatment, mortality prognosis and medicinal agents demand. It was designed the project of the national consensus for somato- and somatomammopropinoma diagnostics and therapy. The creation of algorithm for acromegalia diagnostics will enable us to standardize the delivery of medical service for this endocrinopathy in Ukraine. Product Description popup.authors 1.
