Object of study: 267 patients with saccular aneurysms (SA) of brain vessels, operated by cause of rupture of SA which was complicated by angiospasm( AS). Purpose of study: to develop new approaches of endovascular treatment of patients with SA in early period, which was complicated by AS. Methods : standard survey, analysis of physical data and neurological status, digital subtraction angiography(DSA), computed tomography (CT), magnetic resonance imaging(MRI), ultrasound of brain vessels. Results: were studied features of AS in patients with ruptured SA. It is established that AS was in 83% patients with ruptured SA and 12.4% patients did not have AS. The main reason of AS in ruptured SA is the volume of spontaneous intracranial hemorrhage (SIH). Patients with SIH by Fisher 2-3 - SA occurred in 37.5%, in patients with SIH by Fisher 1- only in 4.2 % patients. There are no reliable facts that AS developing more often in of this group ( SIH, subarachnoid-ventricular hemorrhage (SVH), subarachnoid-parenchyma hemorrhage(SPH)). The repeat ICH when AS is present in 100% cases leads to it critical development and high risks of disability of the patient. Pharmacological and mechanical angioplasty is effective method to influence on AS. The intraoperative usage of mechanical and pharmacological angioplasty allows coil SA even in early rupture period . Mechanical angioplasty has more benefits in treatment of local AS 1-2 types( by A.A Krylov) even when artery occlusion more than 50 %. The usage of pharmacological angioplasty with coil occlusion of SA is an effective method. Superselective pharmacological vasodilatation with solution of papaverini is more useful when artery occlusion more 70% because of technical benefits and somplicity. The method of treatment and prophylaxis of SA was developed with usage of modern endovascular techniques depending on type of SA (Patent Request № U201108232 since 01.07.2011). Effectiveness: Integration in medical practice results of investigation improve efficiency of treatment patients with ruptured SA of the brain vessels in early period with AS and decrease mortality and disability of the patients.
