Purpose: Оptimizаtion of the diagnosics and treatment of the patients with recurrent nodular euthyroid goiter (RNG) on the basis of risk factors determination. Methods of study: general clinical, enzyme immunoassay, standard histological, ultrasonography (USG) of thyroid, methods of formal genetics, statistical methods. Retrospective analysis revealed a steady level ratio of female / male patients with postoperative recurrent goiter (15,5:1). Macrofollicular colloid goiter, adenomas of thyroid, Hashimoto's thyroiditis had the highest frequency among the recurrent thyroid disease forms. Papillary thyroid cancer forms were found in six patients (4.11%). The study of thyroid hormone status in the patients with RNG revealed a high incidence of postoperative hypothyroidism after standard surgery (76%). The use of intraoperative dose cryaction led to a slower extinction of thyroid function in the patients with RNG. Local cryoaction led to a significant reduction in antithyroid aggression during 6 - 24 months after surgery. According to the ultrasound RNG patients revealed that nodal tumor formed at the same rate as in the operated thyroid lobe and in the contralateral. The most characteristic features of single-node recurrence of goiter were oval tumors (49.11%), sharp contours (60.71%), decreased echogenicity (66.96%), absence of calcifications (84.82%), lack of liquid component (68, 75%), a clear capsule (55.36%). Criteria for the use of intraoperative ultrasound and a protocol to use were developed.
