It has been show that morphologic substrate in acute period is osteolysis; its expressiveness and speed stipulate hypercalcemia and heterotopic ossification of paraarticular tissues at the area of injured joint. Osteolysis and hypercalcemia are distinctive features during acute period of DOA therefore hypercalcemia can manifest diagnostic sign of acute period and the marker of osteolysis. Hypercalcemia is manifested with normal indexes of phosphorus, alkaline phosphatase of blood and absence of changes in excretion of calcium and phosphorus in urine. Change to chronic stage is characterized by normalization of calcium level in blood. As a result of immunologic study a low regenerative potential of bony tissue has been shown in patients with diabetic osteoarthropathy. However it was noted definite local retention of osteogenic activity of stem stromal cells of bone marrow in acute stage. It has been shown oppression of anabolic processes in osseous tissue of patients with diabetic mellitus who has osteoarticular lesions of foot; metabolism of articular cartilage with diabetic osteoarthropathy is characterized by sharp decrease of collagen content and increase of glycosaminoglycanes. Operative treatment of diabetic osteoarthropathy of foot during acute period of pathologic process, directed to total synovectomy, removal of free osteochondrous fragments, arthrodesis of insured joint with application of stable internal fixation accelerates time-study of the process, provides a good anatomo-functional result (75-91 scores in accordance with the scale AOFAS).
