Objective: to elaborate grounded approaches to medicosocial expertise and invalid rehabilitation due to CRD to increase their life quality regarding individual particularities of the patient. Methods: historic-archival, sociological, clinic functional, laboratory-biochemical, statistic mathematical, structure logical. The quota of invalids was estimated by MSEC of Vinnitsa, Kherson, Khmelnitsky, Rivne regions in 2006-2008 yr. Medicoexpert documentation of 5140 invalids was analyzed. The present state of primary and acquired invalidism because of CRD was studied. Deep clinic functional and laboratory biochemical investigation of 137 patients with renal disorder were carried out in the RI clinic of invalid rehabilitation. To determine markers of residual renal function in CRD VD stage 83 patients undergoing hemodialysis and dialysis were examined nephrological centre in the Pirogov regional hospital. Renal diseases were found to be common cause of the primary invalidism, its level for the last years stays stable and has trend to decrease in working capacity age people. In the structure of the primary invalidism because of CRD the third invalid group prevails specific weight of which gradually decreases of the expense of more serious invalid groups. The level and structure of the acquired invalidism in the course of time practically are not changed. In the follow-up examination in 85-86% of patients invalidism group does not changed. The invalid quota due to renal diseases mainly consists of working capacity age subjects, female, the third group invalids. Chronic glomerulonephritis results in invalidism in a third of cases, chronic pyelonephritis involves a fourth of cases, urolithic diseases takes the third place, congenital renal defects and polycystosis, in particular, are the commonest causes of invalidism. The invalids because the renal diseases are in need of different kinds of rehabilitation: more than 50% of patients need hospital restorative treatment and psychotherapeutic correction, the majority of patients need ambulance restorative treatment, more than 50% of patients request finding situation; social service as measure of social rehabilitation plays a great role as well. Clinic functional particularities in renal diseases which result in vital activity restriction are as follows: progressing disorders of nitrogen secreting renal function, arterial hypertension, heart deficiency development and psychological disadaptation - in a great number of invalids the impairment of emotion-willed sphere and signs of asthenic state are defined. Diagnostic and prognostic value of NT pro BNP level was grounded in CRD VD stage patients who are treated with the program hemodialysis and its connection with the residual renal function, oligoanuria prolongation, structure-functional heart changes and proinflammatory cytokines. On the ground of conducted complex clinic functional investigation, studying social and professional status unique medicosocial criteria were formed. Principles of medicosocial rehabilitation depending on the determined rehabilitative potential in invalids with different forms of chronic renal diseases, principles of forming program of medical, social, professional, rehabilitation were elaborated.
