The aim of our work - to improve the effectiveness of treatment and quality of life of patients with invasive bladder cancer (BC) by use of neoadjuvant chemotherapy and study its impact on the anatomical and histological structure of tumors of the bladder. Methods: clinical, laboratory, X-ray radiological, endoscopic, instrumental, pathomorphological, statistics. We are found that neoadjuvant chemotherapy in the treatment of patients with invasive BC allowed in 9.8 % cases get complete response, and in 58.5 % - a partial regression of tumors by RECIST criteria. Detected relationship between the degree of tumor differentiation (G) and the degree of tumor regression under the influence of neoadjuvant chemotherapy ((G1 - (84 ± 10,8) %; G2 - (55,8 ± 31,7) %; G3 - (32,1 ± 35,3) % according to the RECIST criteria). Holding of neoadjuvant chemotherapy allowed in 60% of cases improve or normalize renal function and urodynamics of upper urinary tract, significantly reduce the volume of the prostate, improve urinary act and quality of life of patients with invasive BC. Detected a direct relationship apoptosis index increase the degree of tumor regression under the influence of neoadjuvant chemotherapy (p < 0.05) allows to recommend the use imunoflyuorestsentns studies using confocal microscopy as a criterion for prediction of chemotherapy effectiveness in patients with invasive BC. Shown that the prostate sparing cystectomy in patients with invasive BC with a low risk of invasion into the prostate gland leads to significant improvement in functional outcomes, ensuring the preservation of erectile function in 82.4 % of cases with an average level of IIEF - 5 in the (16,1 ± 4,5) points and the possibility of daily urine retention of 100 % and night - 88.3 %. It is shown that neoadjuvant chemotherapy in patients with invasive BC significantly increases the rate of general 2 - year survival of 20 %. Shown that the combined spinal-epidural anesthesia and intra operative hemodilution creates conditions of moderate hypotension and normosystolic that helps to reduce blood loss during radical cystectomy with (830 ± 241,8) to (515 ± 241,9) ml compared with general anesthesia (p = 0.002). The results of the research are introduced in the National Cancer Institute, Vinnitsa regional oncologic dispensary, Kyiv regional oncologic dispensary.
