After their urgent histological examination the volume of the dissection can be determined which influences the increase of radical surgery. Studied long-term outcomes of patients with gastric cancer after dissection in volume D2 and omentektomii. The results of the randomized multicenter study showed long-term better outcomes of patients after D2- in comparison with D-1 dissection. Using of the proposed methods increases 3-year survival rate by 18% and 13% respectively. High sensitivity and specificity of the markers p53, bcl-2, VGEF and 8-oxodGu can be used for individualization of the treatment for gastric cancer patients. Determination of sensitivity to chemotherapy allows patients to individualize treatment of advanced gastric cancer, increase the number of partial regression in 2.5-3 times and reduce the overall cost of treatment. The use of antitumor vaccines in the combined treatment of patients with resectable gastric cancer increases 3-year survival rate to 13% comparing with surgery alone. Results of the combined treatment of metastatic colorectal cancer patients with R0 surgery and adjuvant chemotherapy were shown. To increase the effectiveness of treatment is possible due to the selection of groups of patients with unfavorable factors, and regimes of individualization of chemotherapy. The use of molecular tests and determine the proliferative potential of the tumor allowed to individualize adjuvant chemotherapy regimes, increase the duration of relapse-free period to 30%. It is possible to personalize the adjuvant treatment and improve survival rate of the colorectal cancer patients with liver metastases. FOLFIRI/FOLFOX regimens is effective as adjuvant treatment. 3-year survival rate was 94%. Results of the randomized study of the comparing neoadjuvant short-course or long-course radiotherapy at the resectable rectal cancer patients are presented. Combination treatment of patients with resectable rectal cancer with the use of neoadjuvant radiotherapy (long course) is an effective method that improves by 2.5 times the number of sphincter saving surgery and improve the quality of life of patients. In 38% of patients with objective treatment effect came in the form of partial or complete tumor regression. Application of the proposed method of combined treatment improved the 3-year disease-free survival rate by 13%. Randomized trial found that use of a multimodal program "Fast track surgery" (FTS) comparing to conventional method increases the effectiveness of surgical treatment of colon and gastric cancer patients. Due to the combined use of epidural anesthesia, minimizing surgical intervention, early activation and nutrition program FTS reduces pain in 3 times and rapid recovery of the gastrointestinal tract. Application of a FTS program significantly reduces terms of rehabilitation and costs of treatment. The results of research implemented in the Nicholas, Chernigov, Poltava, Chernivtsi oblast and Sevastopol City oncology clinics. Results of research put into practice in the Mykolaiv,Chernigyv, Poltava, Chernivtsi regional and Sevastopol city cancer hospital.
