The aim. The estimation of the state of epithelium of tumor- distant oral mucosa аnd biology of basal cells dependent on histopathological alteration of oral mucosa in patients with oral squamous cell carcinoma (OSCC) and epidemiological factors of risk . Materials and methods. The biopsy specimens of tumor-distant oral mucosa and tumors were obtained from 18 patients with OSCC (male, age from 43 to 79 years). The alteration of oral mucosa and tumors were classified according to the UICC criteria. Immunohistochemically were used expression of regular nuclear marker of the epithelial stem /progenitor cells protein p63 and biomarker proteins p53, Ki-67, Bcl-2 in epithelial layers of tumor- distant oral mucosa. Tissue sections were immunohistochemically stained using monoclonal antibodies: for р63 (clone 4A4, antibody to all isotype ), for p53 (clone DO-7, antibody to wild/mutant type), for Ki-67 (clone МІВ-1) for Всl-2 (clone 124), and En Vision "Daco Cytomation" (Denmark) and counted according to epithelial layers as Labeling Index (LI %). Results. Most of the patients (83,4%,15/18, і 20 years і 20 cigarettes per day) have the tоbaсco smoking - drinking status and long-term professional contact with carcinogen and mutagen. In tumor-distant oral mucosa revealed the progression of histopathological phenotype. There were: hyperplasia (2), dysplasia mild ( 2), dysplasia height ( 3) Сa in citu ( 4), invasive carcinoma (7).The study of profile expression of marker protein p63 and biomarker proteins р53, Кі-67, Всl-2 in epithelial layers of tumor-distant oral mucosa showed revealed the various level and combination of they expression in epithelial layers depending on histopathological alterations in mucosa. Negative expression of p53 and predominantly positive expression p63 in cells of basal layer were found in tumor_-distant oral mucosa in cases of hyperplasia ( range LI(%) p63 ,basal layer, 33,8%-52,5%; suprabasal layers 0%-0,2%). p63 expression in the specimens of height dysplasia and dysplasia with areas atypia ( range LI(%) р63 ,basal layer, 12,0%-30,0%; suprabasal layers, 40,3%-70%), Сa in citu (range LI(%) р63, basal layer, 10,3-,30,4%; suprabasal layers 10,0%-30,0%), invasive carcinoma ( range LI(%) p63, basal layer 3,0%-35,6%; suprabasal layers, 13,8%-65,7%; in areas invasive carcinoma, 41,7%-52,9%) was significantly increased in suprabasal epithelial layers. This results indicated that anomal suprabasal expresion of marker stem/progenitor cells p63 may play role in formation of the transformation phenotype. Conclusion. The profile expression and Labeling Index (LI %) of marker protein p63 and of biomarker proteins р53, Кі-67, Всl-2 in cells of suprabasal epithelial layers of tumor-distant oral mucosa correlated with heigh -grade epithelial dysplasia and formation of areas of transformation phenotype.
