The aim. Investigation of the alteration of oral mucosa in individuals with epidemiological risk of oral cancer, in precancerous lesions , in tumor-distant mucosa in patients with OSCC. The compare of the expression of of regular nuclear marker of the epithelial stem /progenitor cells p63 and keratin CK5/14 and histotopography of p53 CK5/14 cells depending on histopathological alterations in oral mucosa and expression of biomarker proteins p53 ,Ki-67.Materials and methods. The biopsy specimens of oral mucosa from individuals that nonsmoking tobaсco (control group,13cases), smoking tabaсco (groups of epidemiological risk,і 20 years і 20 cigarettes per day, 13cases), of precancerous lesions ( 41cases); and biopsy specimens of tumor-distant oral mucosa from patients with oral squamous cell carcinoma (OSCC).The alteration of oral mucosa and tumors were classified according to the UICC criteria. Immunohistochemically were used expression of regular nuclear marker of the epithelial stem /progenitor cells protein p63, keratin CK5/14 and biomarker proteins p53, Ki-67 in epithelial layers oral mucosa. Tissue sections were immunohistochemically stained using monoclonal antibodies: for р63 (clone 4A4, antibody to all isotype ), for CK5/14 (clone124 ), for p53 (clone DO-7, antibody to wild/mutant type), for Ki-67 (clone МІВ-1), and En Vision "Daco Cytomation" (Denmark) and counted according to epithelial layers as Labeling Index (LI %). Results. Basal cells of normal epithelium of oral mucosa strongly expressed the marker proteins of epithelial stem /progenitor cells p63, CK5/14 as the predominant (control group .p63,range LI(% )52,4 % -66,3% ; С5/14,range LI(%) 50,2%-56,0 %). The basal cells of oral mucosa of individuals of control group were negative expression of p53 and positive expression of Ki-67 in cells of basal/ parabasal layers. In oral mucosa of individuals with epidemiological risk revaled the of histopathological progression. There were: hyperplasia (2 cases), dysplasia low (2 cases) dysplasia mild (2 cases ), dysplasia height ( 4 cases)/ The study of profile expression of marker protein p63 С5/14 and biomarker proteins р53, Кі-67 in epithelial layers of oral mucosa from individuals of group of risk showed revealed the various level and combination of they expression in epithelial layers depending on histopathological alterations in mucosa and anomaly distribution in suprabasal layers of height dysplasia (.63,range LI(%) 37,2%-56,3% ; С5/14,range LI(%)20,5%-46,6%) . The profile and Labeling Index (LI %) of expression of markers of the epithelial stem /progenitor cells p63, CK5/14 biomarkers p53, Ki-67 in cells of suprabasal layers oral mucosa from individuals of risk group correlate with height dysplasia and formation of areas transformation phenotype. This data to confirm the participation of basal cells with expression of markers of the epithelial stem /progenitor cells p63, CK5/14 formation of areas transformation phenotype. In precancerous lesions, in tumor-distant mucosa revaled the progression of histopathological phenotype (hyperplasia, dysplasia (low, mild, height),carcinoma in situ, invasive carcinoma) and expression of marker proteins p63, CK5/14 and biomarker proteins р53, Кі-67 in suprabasal layers of epithelium. The suprabasal layers expression of proteins p63, CK5/14, p53 correlated with height grade of epithelial dysplasia and risk of oral cancer. Conclusion. Its to confirm the participation of cells, that expression of markers proteins of the epithelial stem //progenitor cells p63, CK5/14 of basal layer in formation of transformation phenotype and biology of OSCC. The immunohistochemicall estimation of Labeling Index (LI %) of p63, CK5/14, p53 cells in suprabasal epithelial layers of oral mucosa can be used for monitoring of cancer risk in oral mucosa of individuals with epidemiological risk.
