Investigative techniques - general, clinical, immunologic, immunofluorescence, ELISA, histo-morphological, instrumental, statistical. 774 patients after suffering a CVA and 155 healthy individuals randomized by age and sex and 22 autopsy cases were examined. It was stated, that the clinical course of the disease among patients after ischemic cerebrovascular accident is caused by changes in the immune system and cytokine status and depends on the location of the lesion, the degree of neurological disorders and stage rehabilitation period. The most serious consequences of cerebrovascular accident were observed during early rehabilitation in the presence of moderate and marked neurological disorders and with left-sided localization of ischemic lesions and were accompanied by the highest level of pathogenic CIC, proinflammatory cytokines (TNF-alfa, IL1-beta, IL-6) and anti-inflammatory IL-4 , enhanced by apoptosis lymphocytes, depression of early activation and adhesion processes, the development of B-lymphocytosis, reversion immunoregulatory index and manifestation of maximum activity of the inflammatory process, that is confirmed by the highest levels of IL-6. The high information content and pathogenetic significance were stated in the occurrence and progression of CVA of activated subpopulations of lymphocytes CD25 +, CD54 + and CD95 + lymphocytes, proinflammatory cytokines - TNF-alfa, IL1-beta, IL-6, medium -and - small molecular circulating immune complexes with pathogenic properties which was confirmed (1) - by morphological studies. An effective way to limit the area of inflammation due to restore the normal balance of physiological and pathogenic circulating immune complexes (indicators of immune status and clinical status of patients by reducing the number of patients with severe neurological disorders - F3 and F4) were developed, tested and put into practice. It was stated, that using simvastatin in complex treatment of patients suffering from the consequences of ischemic cerebrovascular accident contributes to evidential reduction processes of adhesion and migration of lymphocytes to atherosclerotic plaques according to CD25 + and CD54 + cells level and normalization of CD95 + lymphocytes and proinflammatory cytokines.
