FD is divided on several types: epigastric pain syndrome (EBS) - 28.13%, postprandial distress syndrome (PDS) - 56.25%, non-specific - 15.63%. Esophago-gastro-pH-metry (monitoring), which were performed on 178 patients with FD and 247 patients in the control group (GERD, peptic ulcers), significantly improved the diagnostic capabilities. In particular, one-quarter of patients FD demonstrates abnormal gastroesophageal reflux, which requires a long-term therapy. 5% of patients have atrophic gastropathy with hypo-anacidity, making impossible traditional prescribing of antisecretory drugs. Basic therapy with proton pump inhibitors (PPI) does not reach the required level of acidoinhibition in quarter of patients and requires conducting of pH-monitoring to adjust the dose of PPI or addition 150 mg ranitidine at night. The sensitivity of patients with FD to PPI is lower than in peptic ulcers , significantly increases in case of HP- infection and doesn't depend from the type of PPI. In the subjective resistance to PPIs it is worth to prescribe 500 mg "Phenibut" BID, which reduces anxiety, normalizes autonomic dysfunction and eliminates depression to the 30-th day of treatment . This therapy is indicated for FD-patients overlaped with IBS (up to 34.7% of FD). In case of FD-PDS PPI's efficacy obvious increases by adjunctive taking of 10mg domperidone TID.
