The object of study: 126 patients with chronic lymphocytic leukemia (CLL) and 242 individuals without oncohematological pathology. Methods: the determination of rs966221 of PDE4D gene polymorphism by polymerase chain reaction and restriction of products, clinical, statistical. The main results: the frequencies of rs966221 polymorphisms of PDE4D gene were determined among patients with CLL and persons without oncohematological pathology (control group) who are constantly living in Ukraine. It was found that distribution of polymorphisms in controls did not differ depend on radiation anamnesis, gender, and age of observed patients and corresponded to the others groups of Caucasians. The impact of these polymorphism on the risk of CLL in patients exposed to ionizing radiation (IR) did not found. Some differences were revealed among IR-exposed and IR non-exposed CLL patients, namely decreasing the CC genotype carriers among IR non-exposed CLL patients, and association of CC genotype with unmutated IGHV genes, first of all, 3 family IGHV genes, among IR-exposed CLL patients. Some associations of rs966221 of PDE4D gene's genotypes with clinical features of CLL were revealed: low efficacy of second line therapy (p=0.03); tendency to more frequent development of autoimmune anemia (p=0.09), expecially in unutated IGHV genes (p=0.012), and cytopenias after chemotherapy (p=0.073), an absence of Richter transformation (p=0.101) in carriers of TT genotype in comparison with carriers of others genotypes. In the control group of IR-exposed individuals it was found lower number of CC homozygotes among patients with ishemic heart disease (p=0.015) in comparison with carriers of others genotypes.
