The object of study - 617 patients with renal cell carcinoma. The aim of the study - to reduce disability, improve diagnosis and treatment of kidney cancer improving indications, operative techniques and requirements for organ-sparing surgeries. Methods: Clinical, laboratory, radiation, statistical. Patients with renal - cell carcinoma founf to have chronic renal failure in 73.1% of cases with comorbidities as hypertension, diabetes or obesity, which also affect kidney functional status and is an indication for organ-sparing. It is shown that the size of kidney tumors more than 7 cm statistically significantly reduces glomerular filtration rate (GFR) by 12.8%, while the number of patients with symptoms of chronic renal failure progressively increases with increasing stage of disease (p <0.01). Resection of the kidney provides a significant increase in the level of total GFR by maximizing storage capacity of functional parenchyma (p <0.0001) and a decrease in the development of chronic renal failure in the postoperative period by 42.9% compared with radical nephrectomy (p <0.001) which leads to disability of patients. The method of assessment of nephrometric indicators in renal - cell carcinoma was developed, which allows to determine the amount of functional renal parenchyma on the affected side and shows an adequate indications for surgical intervention. It was found that in 99.3% of patients with renal - cell carcinoma who underwent resection of the kidney, the minimum amount of functional parenchyma on the affected side was more than 55%. We developed a setting of actions to implement the kidney resection in patients with renal - cell carcinoma, they include: transabdominal approach, finding and control of renal major vessels, local ischemia, tumor removal while preserving the maximum amount of parenchyma in combination with a complex kidney protective therapy that reduces the risks of violating kidney function in future.
