The purpose: to define a complex of genetic markers of predisposition to development of a multifactorial pathology (endocrynological, hematological, oncological) at clean-up workers of the accident on CNPP and members of their families for predisposition of negative medical consequences in kept away the postaccident period. Methods: clinical, immunogenetic, methods of clinical genetics, cytogenetic, dermatoglyfic, methods of mathematical statistics. Groups of priority supervision on nozological forms of multifactorial diseases which can causally be connected with an irradiation (tyreoid, oncohematological and cardiovascular pathologies) are generated. It is established, that specificity HLA of markers of genetic predisposition does not depend on presence or absence of influence of the radiating factor and is the general within concrete studied nozological disease forms. Genotype of markers of genetic predisposition to disease in a complex with markers of radio sensitivity of an organism considerably raises risk of realisation of disease in the conditions of radiation faktors. To occurrence risk factors oncohematological diseases are carried HLA-A*24; HLA-A*02; HLA-A*11; HLA-B*18; HLA-B*35; HLA-B*51; HLA-C*03; HLA-DRB1*11 and HLA-DQА1*0101, DQB1*0501 hyplotype, HLA-А*29; HLA-В*38; HLA-В*21; HLA-DR3,7; HLA-DR5, DQ1 alleles are immunogenetic development markers of tyreoid pathologies. To risk factors of occurrence of ischemic diseases of heart are carried HLA-А*29 and HLA-А*33, HLA-DRB1*12, HLA-DQA1*0103, HLA-DQA1*0501 alleles, and hyplotype combination HLA-B*35, HLA-DRB1*12. Specificity: HLA-DRB1*04, HLA-DQA1*0201, HLA-DQB1*0602 carry out protective function concerning risk of realisation of ischemic deseases of heart irrespective of comparison group. Meangroups cytogenetic and molecular-cytogenetic indicators of children of clean-up workers of the accident on CNPP 1983-1985 and 1988-1995 of a birth did not exceed meanpoppulations values and fluctuated within a method error. Features of a clinical current of disease at patients with a multifactorial pathology with the account initial semiology, a stage of disease, presence of complications in comparison to a complex of genetic markers have laid down in a basis of working out of a complex clinical, immunogenetic and dermatoglyfic criteria of diagnostics and current predisposition of oncohematological diseases, tyreoid and cardio - vascular pathologies, including radioassociation.
