Purpose of the study: to develop and implement approaches to diagnosis, treatment and prevention of metabolic syndrome in patients with AH. Methods: the systematic analysis, questionnaire, statistical, clinical, instrumental and laboratory. Results. The prevalence of the metabolic syndrome (MS) according to ATP III criteria was determined during the screening of the random sample of public servants in the age up to 60 years. MS was verified in 42,86% of male patients and 41,46% - in females. Co-existence of abdominal obesity, hypertension and increased triglycerides was the most common combination of MS components. The evaluation of diets in civil servants with hypertension and metabolic syndrome was performed. The unbalanced income of some nutrients was determined: increasing of amount of easily digested carbohydrates and saturated fats, lack of complex carbohydrates, some vitamins and minerals. This imbalance is the unfavorable background for the correction of metabolic disorders. The extrapyramidal motor deficits of different severity and signs of cognitive, emotional and volitional disorders as consequences of the phenomenon of metabolic diaschisis were observed in patients after ischemic stroke. The degree of extrapyramidal motor deficit directly correlated with the amount of the affected brain tissue, what was of practical value and justified the pathogenetic treatment and prevention of stroke. It was shown that concomitant hypothyroidism was a significant factor for poor prognosis of ischemic cerebral complications (hypertensive dyscirculatory encephalopathy) with cognitive changes and anxiety-depressive disorders. This justified the need of optimizing the prevention, treatment and rehabilitation. The clinical course of AH in patients with hyperuricemy and gout was studied. It was determined that hyperuricemy in hypertensive men was a significant risk factor for cardiovascular complications and associated with increased incidence of myocardial infarction irrespective of presence of gout. AH with concomitant gout in men contributed to the early renal damage and lead to significant reduction of glomerular filtration rate. It was proved that erythrocytosis in patients with AH was a sign of high and very high cardiovascular risk patients. The particular clinical signs of such patients are excessive body mass index, high levels of diastolic blood pressure, cholesterol, uric acid and serum erythropoietin.
