The object of study - patients with chronic secretory otitis media (CSOM) with retraction pockets (RP), retraction pocket, matrix of cholesteatome, mucosa, bone. Purpose - to improve treatment of CSOM with retraction pockets by studying the features of the pathogenesis of development and further development of methods of diagnosis and treatment. Methods - clinical, audiometry, vestibulometry, spiral computed tomography (SCT), craniometry, biochemistry, electron microscopy, analytical and statistical. The features of CSOM with RPs, depending on their shape, condition (unstable, stable), the ability to cleaning itself. Developed a diagnostic algorithm used to determine the shape, condition and stage of RP. Defined diagnostic capabilities SCT in the recognition and diagnosis of CSOM with RPs, which was a subsidiary method in choosing a method of treatment of patients with various forms of RPs and identify signs of relapse process and assess the state of mastoid cavity and middle ear ventilation in operated patients in the late postoperative period. The schemes of treatment, depending on the form and stage of retraction pockets. In the first stage all patients were surgically treated by procedures to improve the function of the eustachian tube. Patients with mobile RPs in the presence of fluid in the tympanic cavity should be treated by ventilation tube inserted to the health part of tympanic membrane. In patients with fixed and controlled RPs tympanoplasty with dense tympanic graft as a fascial flap, reinforced by cartilage was performed. In patients with fixed uncontrolled RPs and precholesteatome state able to a tympanoplasty, as described above, while the T-shaped ventilation tube into the groove formed in the inferior wall of the bony external auditory canal was performed, and the posterior timpanotomy with draining tubes through which the solution of aprotinin in a dose of 1.0 ml (10 000 AtreU) injected for 7 days. Ossiculoplasty performed either simultaneously or delayed. Designed scheme of combined treatment was used in 94 patients. The results of treatment of patients in the early and late follow-up periods evaluated. Defined biochemical signs of RPs with determination of their enzymatic activity and electron microscopic characteristics of various forms of RPs in CSOM.
