In both age groups, insulin resistance (IR) has promoted development of prothrombotic changes of the homeostatic system mainly due to inhibition of the fibrinolysis, as evidenced from an increased concentration of the plasminogen activator inhibitor. Notably, the disturbances of the fibrinolysis were more significant in the elderly versus middle-aged people with IR. In addition, seniors with IR had a lower indicator of the activated partial thrombin time (APTT), indicating more significant prothrombotic changes in the hemostasis. Manifestations of above-mentioned changes were even more pronounced in the patients with impaired glucose tolerance. With IR, the vasomotor endothelial function was more decreased in middle age, especially in the presence of impaired glucose tolerance. At the same time, the elderly IR leads to a more significant disturbance of the anticoagulant activity of the endothelium. In both age groups, IR was accompanied with increased levels of the proinflammatory cytokines (IL-6, TNF) and C-reactive protein, indicating the formation of proinflammatory phenotype. Older people with IR displayed definite vascular, extravascular and intravascular changes in the microcirculation of the bulbar conjunctiva, an increased overall conjunctival index, being indicative of the microcirculation deterioration. The elderly with metabolic syndrome receiving course melatonin treatment have showed the decrease of insulin resistance, improvement of carbohydrate metabolism (normalization of impaired glucose tolerance) and lipid composition of blood, endothelium functioning and blood rheology. We can recommend melatonin for complex correction of the manifestations of insulin resistance in elderly patients.
